and insurance details. Our staff is dedicated to ensuring paying for your services
is as convenient as possible. Billing information and direct phone numbers
are provided on our site. If you still have questions or concerns about
your insurance or billing, please contact Story County Medical Center at (515) 382-2111,

Most insurance company ID cards have a customer service
phone number on the back. Before you call, have available your insurance card, date
of service, facility name, original billed amount, patient name and claim number
if applicable. Write down the name of the person you talked to at the insurance
company. If the bill has not been paid, find out when the anticipated payment date
is, and ask what is needed. If the bill is not paid in the stated timeframe, follow-up
with the insurance company again and, if necessary, request to speak to a supervisor.

I would like to obtain a copy of my personal
medical records. What do I have to do and who should I contact?
The Medical Records office is
the main area of storage for medical documents which contain information concerning
all patients' subsequent hospital visits. These files help establish past conditions
or issues that may influence your current or future treatment at Story County Medical
Center.
You may obtain copies of your records for personal use and information, in accordance
with HIPAA regulations. To obtain a copy of your personal records, please
contact the Medical Records Department at (515) 382-7017. You may print the
Authorization to Release Patient Information Form
and fill it out in advance to speed up the process.
