Story County Medical Center’s Diabetes Education Program
assists people with diabetes to live healthy and enjoyable lives.
Whether you have just been diagnosed with diabetes or have had diabetes for years,
self-management is the key to living a healthy life. Story County Medical Center
diabetes specialists work with you, your family and your physician to develop a
self-management plan that meets your individual needs. You will learn the skills
and strategies needed to control your diabetes and live a healthier life.
Education is the starting point for successful self-management. We help you take
charge of your diabetes by providing the following education programs:
• Diabetes and how it affects your body
• Developing an exercise program that is right for you
• Personalized meal planning
• Minimizing and avoiding diabetes-related complication
• Blood glucose monitoring for better disease management
• Managing your medications
• Promoting skin and foot health
This program has been awarded recognition by the American Diabetes Association in
accordance with the National Standards for Diabetes Patient Education Programs.
Most insurance plans and Medicare cover Story County Medical Center’s
diabetes education program, however a referral by your physician and program registration
is required. |
